Peak Performance reception staff is available to assist you in starting your road to recovery with Peak Performance. If you have a question about Physical Therapy the front desk staff will direct you to the appropriate clinician when and if they do not have the answer.

Peak Performance clinicians will help you restore ROM, strength, agility, coordination, balance, and power to allow for a return to an independent, pain-free, and functional lifestyle. Our treatment will always be directed at restoring your quality of life.

Peak Performance can recommend a doctor, or you can have your own doctor analyze your condition to determine if Physical Therapy is what you need to recover. Peak Performance Intake Forms are available on this website for you to print out and fill out prior to your first visit. Bring these filled out forms, the prescription from your doctor, ID, and your insurance card to your first visit. Please wear loose fitting clothes and be aware that the injured area will need to be exposed during the evaluation. For example, if you have a knee injury, then shorts that expose your knees will be most appropriate.
